Before any child may participate in this program the parent must complete and return all forms. I must have these forms fully completed and, in my possession, before I can accept the responsibility of caring for your child. No Exceptions!

• Physician’s Report

• Permission to Photograph

• Noah’s Ark Quality Academy Agreement Form

• Health & Safety Code Requirements Form

• Parent Handbook Acknowledgment Form

• Getting To Know Your Baby

• Non-Paying Parent Form

 

Copies:

• Child’s Birth Certificate

• Child’s Immunization Record

• Both Parent's Identifications- any other person that will be picking up the child

 

Upon Arriving Additional Items Needed:

*All Items need to be labeled with the first name and the first letter of the last name*

 

• Earthquake Kit

• 2 changes of clothing (one for colder weather & one for hot weather)

• Extra pair of socks and underwear

• A bag of diapers and/or pull-ups/4 changes of underwear- if potty training, if not 2-Bottles w/top

• 2 boxes/ bags of wipes

• Small brush for hair

• Toothbrush
